Cosmetic Injector & Board Certified Nurse Practitioner (current certifications from both AANP & ANCC.
Masters of Science in Nursing: Family Nurse Practitioner; Graduate of the University of Colorado-2012.
Post Masters Certificate in Adult Gerontology/Acute Care, UC-Anschutz Medical Campus – 2015.
She then went on to complete an additional 100 hours of Advanced Hormone Management training with The Academy for Preventive and Innovative Medicine.
Nurse Practitioner assignments have included areas of Family Practice, Gerontology, Interventional Radiology, Corrections, and Acute Care. Registered Nursing roles included Cardiac Telemetry, Varicose Vein Outpatient Surgery, and Aesthetics. She was first introduced to cosmetic injecting & laser therapy in 2010 and became certified in Botox, fillers, and laser therapy.

Candi is passionate about helping patients feel their best, inside and out; with an emphasis on patient education and fostering patient-centered relationships to help patients achieve their wellness goals. She believes that many of the limitations we perceive to be keeping us from our goals are self-imposed and with thoughtful partnering and guidance she can help her patients reach their best version of themselves. She believes that “everything in life is a practice” and that sometimes we all need a little help knowing what to practice.
She advocates for whole human wellness through nutrition & lifestyle consulting and hormone optimization through bioidentical hormone replacement therapy. Patients will have the opportunity to learn about good skin care habits and how the use of prescription skin care products, microneedling & cosmetic injectables can help them attain their aesthetic desires.
When she is not working with her patients you can find her with her husband and her dogs enjoying the mountains. She loves hiking, fishing, camping, snow shoeing, snowmobiling, and all things outdoors.
Say hello to Dr. Condon! She is coming alongside the Transcend team to add another level of expertise! Dr. Condon has long been intrigued by the many different treatments that can be accomplished with lasers. The aesthetic portion is a unique area to expand her skills and allow her to help people look their best. Dr. Condon belongs to the American Optometric Association (AOA) and the Colorado Optometric Association (COA). She previously served on the COA board and as a key person in a grassroots effort to expand the scope of Optometry. Dr. Condon and her husband enjoy golf, hiking, wine tasting and exploring the area. At Transcend Beauty and Wellness, she performs laser & microneedling services, botox, and filler.
Say hello to Jessica Carlson RN! Jess is a Certified Laser Specialists, Certified Cosmetic Injector, and IV therapy nurse at Transcend! She graduated in May 2022 with her AAS in Nursing from CMU. She had previously worked in education for 15 years before going back to nursing school. She also holds a BA in Kinesiology with an emphasis in Health and Wellness. She has worked Telemetry, ICU and is now in the Emergency Department at Delta Health. Outside of work, Jessica enjoys reading, traveling and spending time with her family and friends. She has been married to her husband, Derek, for 26 years and they have three children (Elias, Abby and Jillian) who are all serving in the United States Military. At Transcend Beauty and Wellness, Nurse Jess assists with in-office procedures and performs laser & microneedling services, botox, and filler.
Sophie Patenaude, Care Coordinator, is a dedicated healthcare professional with a background in patient care and behavioral health. Originally from New England, she moved to Colorado in 2021. Sophie currently works in Patient Access at Delta Health, leveraging her clinical knowledge and Bachelor of Science degree in Criminal Justice. Prior to this, she served as a behavioral health clinician. With a passion for continuous learning, Sophie is deeply interested in nutrition, wellness, and the latest advancements in skincare. In her downtime, she embraces the Colorado lifestyle, enjoying traveling, hiking, rafting, baking, and she is a devoted animal lover, especially of cats

We currently accept Medicare with/without supplemental plans, (and with/without United & BCBS Advantage Plans), BCBS, Aetna, TriCare, and United Healthcare (not Rocky Mountain or Monument).
